HOW TO ENTER: To nominate your league captain for a chance with a prize in the 2011 – 2012 League Captain of the Month Sweepstakes (the "Sweepstakes") sponsored by United States Tennis Association Incorporated (“USTA” or “Sponsor”), visit www.nominateyourcaptain.usta.com and complete the Official Entry Form as indicated. Entry requires that you submit information about you and about your league captain, including a statement about why your league captain is the best (statements have no required minimum length, but must be no more than 800 characters). Nominated league captains must be at least eighteen (18) years old at time entry is submitted. Once you have completed the Official Entry Form, click the "submit" button to complete the entry. The Sweepstakes begins at 12:00:00 a.m. (ET) on July 1, 2011, and ends at 11:59:59 p.m. (ET) on June 27, 2012 (the “Promotion Period”). The Sweepstakes is comprised of twelve (12) separate monthly entry periods (each, a “Monthly Entry Period”). Entries from one Monthly Entry Period will not be included in the pool of entries for any other Monthly Entry Period. Limit one (1) entry per person/per email address/ per household over the entire entry period. All entries become the sole property of USTA and will not be acknowledged or returned. Entries will be declared made by the authorized account holder of the e-mail address submitted at the time of entry. “Authorized account holder” is defined as the natural person who is assigned to the e-mail address by an Internet access provider, on-line service provider, or other organization (such as a business, educational institution, etc.) that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address. The authorized account holder may be required to provide proof of identity. Any other attempted form of entry is prohibited; no automatic, programmed, robotic or similar means of entry are permitted.

ELIGIBILITY: The Sweepstakes is open only to legal residents of the 50 United States and the District of Columbia, who are eighteen (18) years of age or older. Officers, directors and employees of USTA, its affiliates, sections, districts, agents, advertising and promotion agencies or any other agencies or entities involved with this Sweepstakes and members of their respective families (parents, siblings, children, and spouse and their respective spouses) and/or those residing in the same household (whether related or not) of each of the above are not eligible to enter or win. By participating in this Sweepstakes, participants agree to be bound by these Official Rules. USTA reserves the right to disqualify from this Sweepstakes any person it believes has intentionally violated these Rules.

DRAWINGS; ODDS: There will be twelve (12) Monthly Prize drawings as follows:

1. All eligible entries received between 12:00:00 a.m. (ET) on July 1, 2011 and 11:59:59 p.m. (ET) on July 27, 2011 will be included in a random drawing conducted by Sponsor on or about July 28, 2011 to select the “League Captain of the Month” for August;

2. All eligible entries received between 12:00:00 a.m. (ET) on August 1, 2011 and 11:59:59 p.m. (ET) on August 29, 2011 will be included in a random drawing conducted by Sponsor on or about August 30, 2011 to select the “League Captain of the Month” for September;

3. All eligible entries received between 12:00:00 a.m. (ET) on September 1, 2011 and 11:59:59 p.m. (ET) on September 28, 2011 will be included in a random drawing conducted by Sponsor on or about September 29, 2011 to select the “League Captain of the Month” for October;

4. All eligible entries received between 12:00:00 a.m. (ET) on October 1, 2011 and 11:59:59 p.m. (ET) on October 27, 2011 will be included in a random drawing conducted by Sponsor on or about October 28, 2011 to select the “League Captain of the Month” for November;

5. All eligible entries received between 12:00:00 a.m. (ET) on November 1, 2011 and 11:59:59 p.m. (ET) on November 28, 2011 will be included in a random drawing conducted by Sponsor on or about November 29, 2011 to select the “League Captain of the Month” for December;

6. All eligible entries received between 12:00:00 a.m. (ET) on December 1, 2011 and 11:59:59 p.m. (ET) on December 28, 2011 will be included in a random drawing conducted by Sponsor on or about December 29, 2011 to select the “League Captain of the Month” for January;

7. All eligible entries received between 12:00:00 a.m. (ET) on January 1, 2012 and 11:59:59 p.m. (ET) on January 29, 2012 will be included in a random drawing conducted by Sponsor on or about January 30, 2012 to select the “League Captain of the Month” for February;

8. All eligible entries received between 12:00:00 a.m. (ET) on February 1, 2012 and 11:59:59 p.m. (ET) on February 27, 2012 will be included in a random drawing conducted by Sponsor on or about February 28, 2012 to select the “League Captain of the Month” for March;

9. All eligible entries received between 12:00:00 a.m. (ET) on March 1, 2012 and 11:59:59 p.m. (ET) on March 28, 2012 will be included in a random drawing conducted by Sponsor on or about March 29, 2012 to select the “League Captain of the Month” for April;

10. All eligible entries received between 12:00:00 a.m. (ET) on April 1, 2012 and 11:59:59 p.m. (ET) on April 26, 2012 will be included in a random drawing conducted by Sponsor on or about April 27, 2012 to select the “League Captain of the Month” for May;

11. All eligible entries received between 12:00:00 a.m. (ET) on May 1, 2012 and 11:59:59 p.m. (ET) on May 29, 2012 will be included in a random drawing conducted by Sponsor on or about May 30, 2012 to select the “League Captain of the Month” for June; and

12. All eligible entries received between 12:00:00 a.m. (ET) on June 1, 2012 and 11:59:59 p.m. (ET) on June 27, 2012 will be included in a random drawing conducted by Sponsor on or about June 28, 2012 to select the “League Captain of the Month” for July.

Potential Monthly Prize winning league captains will be notified by email and/or telephone on or about the day of the applicable Monthly Prize drawing and may be required to sign and return an Affidavit of Eligibility and a Liability and Publicity Release (where permissible) within seven (7) days of notification. The prize will be forfeited and an alternate winner will be selected at random from the remaining eligible entries and confirmed in accordance with these Official Rules (i) if a potential Monthly Prize Winner does not timely return the requisite documents or cannot be reached within three (3) days from initial notification attempt, (ii) if prize is returned as non-deliverable, or (iii) in the event of non-compliance with any of the requirements of these Official Rules. Limit one (1) Monthly Prize per league captain over the entire Promotion Period.

Sponsor’s decisions are final and binding in all respects. Odds of winning depend upon the total number of eligible entries received during a particular Monthly Entry Period.

PRIZES: Twelve (12) Monthly Prizes:Twelve (12) Monthly Prize winners will each receive a $100 gift card to www.ustashop.com and will be named “League Captain of the Month” and recognized on usta.com. ARV of Monthly Prize: $100, each.

Prizes are not transferable or redeemable for cash. Sponsor reserves the right to substitute a prize for one of equal or greater value if the designated prize should become unavailable for any reason. Winners are solely responsible for all federal, state and local taxes and any other expenses, costs or fees associated with acceptance and/or use of a prize.

GENERAL: By participating in this Sweepstakes, each entrant (and, if an eligible minor, his/her parent or legal guardian) and, by accepting a prize, each Prize Winner (i) agrees to be bound by the terms and conditions stated in these Official Rules, (ii) agrees to be bound by the decisions of USTA and judges, which are final and binding in all matters relating to the Sweepstakes, and (iii) agrees USTA and its, affiliates, subsidiaries, retailers, distributors, suppliers, advertising and promotion agencies, and all of their respective officers, directors, shareholders, employees, representatives and agents (collectively, the “Released Parties”) shall have no liability and will be held harmless by entrant for any claim, action, liability, loss, injury or damage to entrant or any other person or entity, including, without limitation, personal injury or death to entrant or any other person or damage to personal or real property, due in whole or in part, directly or indirectly, by reason of the acceptance, possession, use or misuse of a prize or participation in this Sweepstakes. Entry and/or acceptance of a prize constitutes permission for USTA and its designees to use entrants’ and Prize Winners’ respective name, likeness, performance, hometown and state for advertising and promotional purposes in any and all media at the discretion of USTA without additional authorization, notice or compensation unless prohibited by law. Released Parties are not responsible for lost, interrupted or unavailable network, server, Internet Service Provider (ISP), cable, satellite, website, or other connections, availability or accessibility or miscommunications or failed computer, satellite, telephone or cable transmissions, lines, or technical failure or jumbled, scrambled, delayed, or misdirected transmissions or computer hardware or software malfunctions, failures or difficulties, or other errors or difficulties of any kind whether human, mechanical, electronic, computer, network, typographical, printing or otherwise relating to or in connection with the Sweepstakes, including, without limitation, errors or difficulties which may occur in connection with the administration of the Sweepstakes, the processing of entries or in any Sweepstakes-related materials. Released Parties are also not responsible for any incorrect or inaccurate information, whether caused by site users, tampering, hacking, or by any equipment or programming associated with or utilized in the Sweepstakes. MISCELLANEOUS: Participants agree that this Sweepstakes is governed solely by the laws of the United States and any claims arising out of this Sweepstakes may be brought only in a federal or state court located in the counties of New York or Westchester, New York. If any provisions of these Official Rules are held to be invalid or unenforceable, all remaining provisions hereof will remain in full force and effect.
